Which form of Indian status documentation the patient presented (SCIS, CIS, or TCRD). Rides on the Indian registration number identifier — the same idiom as a health card's version-code extension — because the document form qualifies the presented credential, while the identifier's type stays the identifier's own concern. A recorded expiry on that identifier never implies a registration-status change: a lapsed card does not mean the patient is unregistered.
