Why a care-gap recommendation was not carried out, carried on the statusReason of a
not-done Procedure.
The status reason carries two codings: a code from this system, and a SNOMED CT concept from
R4's procedure-not-performed-reason value set beside it. The SNOMED coding is what an outside
reader interprets; this one is what the recording surface reads back, because the SNOMED concepts
are the nearest available rather than exact equivalents and reading the recorded reason off them
would be an inference. A not-done Procedure carrying only a SNOMED reason was written by some
other path and is not interpreted against this system.
