The printed checkboxes on the provincial laboratory requisition form: which box on the sheet a requested test was ticked from.

Carried on ServiceRequest.orderDetail.coding, alongside a LOINC coding on the same CodeableConcept where the order catalogue has one.

This is a form-layout identifier, not a clinical code. A code here names the position of a box on a ministry form. It is not a test concept, it is not a synonym for any LOINC or SNOMED CT code, and it has no meaning away from that form. A consumer must not substitute it for a clinical code, map it to one by name, or aggregate across requisitions using it as a test identifier. Why it is stored. A reprint has to tick the boxes the clinician ticked. Matching a test to its box by the printed wording would mean an edit to the form's copy silently stops a box being ticked, with nothing failing anywhere. The box identifier is stable; the printed label rides along only as the coding's display.

The list is closed against one revision of one form. Producers write only codes from this list, so it is the complete set a consumer can encounter. It is nonetheless the vocabulary of a single revision of a provincial requisition: a revised form is a new version of this artifact. A consumer reading across versions should tolerate a code it does not recognise.

Two absences are deliberate. Vitamin D has no code here: the form prints no box for it, and the insured / uninsured choice is itself the order, recorded as a form answer. The hepatitis codes apply only once the section's check-one category is "Immune status"; the acute and chronic choices are likewise recorded as form answers rather than as ticked boxes. Finding neither here is not a gap.
