Who pays for the tests on a laboratory requisition -- the payer box ticked on the provincial requisition form.

Carried on Coverage.type.coding, on a Coverage contained in the requisition ServiceRequest and referenced from its insurance element. It is contained rather than referenced out so that a reprint reproduces what the sheet said from the resource alone.

R4's example binding on Coverage.type classifies insurance products; this records something narrower -- which of three printed boxes was ticked -- and is declared locally to keep that correspondence with the form exact.

This is the category, not the paying party. Coverage.payor names who pays. A third-party payer is a box on the sheet, and this system carries no way to identify one; where that identity matters it belongs on payor.

The list is closed: it is the set of boxes the form prints.
