The referral types a decision-support criterion matches on ServiceRequest.code.

This is not the referral's destination. The service or specialty a referral is directed to is carried on ServiceRequest.category, from SNOMED CT (Referral Service Type Value Set). This system names which referral a care-gap criterion is asking about, so that "has this patient been referred for pulmonary rehabilitation in the last two years" can be answered without inferring a programme from a destination specialty.
