Names a chronic-care element on Goal.target.measure where the element has no standard code.
The fallback, not the first choice. Most care elements are LOINC-coded and a goal names the LOINC code, so that a reader outside this platform can tell what is being measured. This system covers the rest -- an element resolved from a vitals catalog key, or a review with no standard place to be recorded -- which have only the identifier their care package gave them.
Component keys are codes here too. A compound reading is bounded per component, so a goal on
one states a target naming the element and one target per component, with the component keys
(systolic, diastolic) drawn from this system. A component key is meaningful only within the
element that declared it and must not be resolved on its own.
Content is not held in this specification. Membership is the set of element ids and component keys the chronic-care packages define, and those are application content revised on their own cadence. This declares the system's identity and defers its content rather than pinning a snapshot. Bind extensible.
When reading a goal, resolve the element from whatever measure carries -- a standard code, a code
from this system, or measure.text. Filtering on system refuses two of the three for no gain.
