ClinicServiceOrganizationIdentifier Naming system

Links an Organization to the tenant it stands for -- the practice as a whole, the party a clinic's Location is managed by. The value is the tenant id, assigned when WELL provisions a store. There is one identifier under this system per tenant, where NamingSystem/clinic-service-location-identifier is one per clinic, so a practice operating four sites has one Organization under this system and four Locations under that one. The identifier carries type v2-0203 XX, an organization identifier with no more specific classification, and the Organization additionally carries a healthcare-provider type, which is what distinguishes the practice's own Organization from the payers and receiving practices sharing the same store. Not to be confused with NamingSystem/organization-identifier, which also sits on an Organization but whose value is an ingest product instance -- the deployment a record was read out of -- rather than a tenant. An Organization may carry both without contradiction and neither substitutes for the other, since one practice can be ingested from several deployments and one deployment can carry several practices.
