PaperChartNumber Naming system

Identifies the number written on a patient's physical chart folder -- the legacy paper chart number, entered by hand before the record was electronic. EMR-LOCAL: a value is unique only within the practice that assigned it, no format is guaranteed, and it MUST NOT be used to match a patient across deployments. It is NOT the medical record number: that is the platform-minted electronic chart number under ../NamingSystem/patient-mrn, and a record may carry both plus a source EMR's own identifier. Identifiers under this system deliberately carry NO Identifier.type -- the v2-0203 code MR belongs to the medical record number, whose consumers match on the type code alone, so this one is identified by its system URI and nothing else. Where a surface wants the chart number a clinician would recognise, prefer this over the medical record number: it is the number printed on the documents in the folder. The URI is stable and is pinned in stored Identifier.system values.
