OnOhipBillingNumber Naming system

Identifies an Ontario OHIP billing number: the provider number the Ontario Ministry of Health assigns to a practitioner who bills OHIP directly, and which Ontario claims are submitted under. Values are the Ministry's own numbers, typically six digits, and are assigned by the Ministry -- this specification neither mints nor validates them. A practitioner without one is normal and conformant: residents, allied-health staff and providers who bill only through a group have none, which is why the slice carrying it is optional. The URI declared here is this specification's own, is stable, and is pinned in stored Identifier.system values. TWO SYSTEMS ARE IN PLAY FOR THIS AUTHORITY AND A READER MUST RESOLVE BOTH. Canada Health Infoway publishes a national naming system for the same number, https://fhir.infoway-inforoute.ca/NamingSystem/ca-on-ohip-billing-id, and the Nexus EMR write path that syncs a provider's record stamps a practitioner's billing number under that URI, so a Practitioner read from Nexus EMR may carry either -- or, on a provider whose record has been through both paths, both. Resolve the URI declared here first and Infoway's second, take the first value found, and treat what you find as one number held under two authorities rather than as two identifiers.
